DDS SUPPORTED LIVING PROGRAM
WORK Inc.'s Supported Living Program provides high-quality case management services to individuals living on their own or in shared homes throughout Greater Boston. The program provides supports for people to live as independently as possible and to assist in achieving each person's personal goals.
A case manager is available to visit individuals at home daily, weekly, or on an as needed basis. The level of support is determined by the person's unique needs.

Admission for the program is primarily through the Department of Developmental Services. The program also accepts private payment for services.
This program works with consumers who have a significant physical disability, and who are also impeded by a sensory, cognitive, learning, or emotional limitation. The program uses the Independent Living Model. The consumer is considered the best judge of what they need, and drives their agenda through their service plan.

•Management of the Personal Care Attendant (PCA) program.
Admission generally occurs from State funding through a contract with the Massachusetts Rehabilitation Commission (MRC) Independent Living Division. Funding is also accepted through private pay, SHIP, and directly through some health care providers.
